Human error blamed for vaccine spoilage

Up to 15-million doses of Johnson & Johnson’s vaccine have been contaminated after a mixup at a plant in Baltimore, Maryland.  The factory produces vaccines for both Johnson & Johnson and AstraZeneca.  The New York Times says workers accidentally combined ingredients for the two vaccines.  The mistake was caught in a quality check  Its unclear how the failed batch will affect future deliveries in the U.S. and to Canada.
