What went wrong with Japan’s reactors?

Reports Monday night say there was another explosion at a nuclear reactor in Japan.

Three reactors at the Fukushima Daiichi nuclear plant lost the ability to cool down.
Crews are dumping sea water into them in a desperate attempt to get the situation under control.
How did the situation get to this point? Nick Dixon looks at what went wrong.
